These states of mood can not be controlled by ones willpower since they have their origin in an imbalanced brain chemistry and are not linked to current life events. This means that somebody with bipolar disorder can be depressed even though nothing bad has happened to him/her.
Many, like Andy Irons are medicating themselves by alcohol and drug abuse. The psychological strain can get really bad for a long time that they would do ANYTHING to make it stop. There is still no cure and sadly quite often (in 20% of the cases) it ends with suicide.
